Multiplex PCR melting curve technology can be used to simultaneously and rapidly detect the resistance of Mycobacterium tuberculosis to the first-line drug isoniazid (detecting katG and inhA genes) and the important second-line drug fluoroquinolones (detecting gyrA gene), and the whole process from nucleic acid extraction to result interpretation can be completed within 2 hours. Current Status
The situation of drug-resistant tuberculosis (DR-TB) is becoming increasingly severe. According to WHO 2024 data, the global isoniazid resistance rate is as high as 15.6%, the fluoroquinolone resistance rate reaches 6.8%, and the fluoroquinolone resistance rate among multidrug-resistant tuberculosis (MDR-TB) patients has risen to 22.4%. The proportion of dual resistance to isoniazid and fluoroquinolones has reached 4.3%, and patients with such resistance face a treatment cycle of 18-24 months with a success rate of less than 50%. However, traditional detection methods have obvious shortcomings: culture-based drug susceptibility testing takes 2-8 weeks, delaying treatment opportunities; most molecular detection methods only cover rifampicin resistance and cannot simultaneously detect isoniazid and fluoroquinolone resistance; moreover, traditional methods are complex to operate, with coverage rate less than 30% at the primary care level. Therefore, developing a molecular detection kit that can simultaneously detect isoniazid and fluoroquinolone resistance, complete testing within 2 hours, feature high sensitivity and simple operation has become an urgent need to achieve precise diagnosis and treatment of TB and prevention and control of drug resistance.
